Insurance and Payments - Billing
Please always bring your insurance cards with you. If you have been in before and have had a change in your insurance coverage or benefits, please notify us when you come in and present your new insurance identification card. Carolina Hearing and Tinnitus participates with Medicare and a variety of insurance plans.
Office visits should be paid at the time of service. We will allow a maximum 90 days for your insurance company's response on claims and will notify you at 60 days when your claim has not been paid. After the 90 days, we will still help you to re-file your claim for your reimbursement, but you will then be immediately and directly responsible for payment of outstanding fees. We will also assist you in pre-determination of benefits and in appeal of any benefits you feel inadequate. If you have questions regarding your insurance, please ask our Insurance Coordinator, Karen Lilly. We will assist in working out financial plans for hearing aids, if needed.
These forms need to be filled out by new patients; they are here on the web so that patients with internet access may fill them out at their convenience instead of at the office.
All new patients must fill out the Patient Registration form.
Patients suffering from other ear ailments also fill out the Patient History form.
